Support that makes a difference

Community Health & Wellbeing Workers

Connecting people to the right help at the right time. Local and practical support to help you navigate everyday health challenges. Friendly, approachable, and professionally trained.

Looking after your health isn’t always easy. Hospitals are busy, and demand on services is an all-time high. That’s where we come in. We provide workforce and support to local organisations, hospitals, councils, and GP federations.

Local Help, Real People

We believe in building stronger and healthier communities. Our team works closely with GPs, councils, charities and local groups so you can get the support you need, when you need it.

We work locally, in your neighbourhood, offering friendly and practical help that puts patients first.

Although our organisation is clinician led, CHWWs are not doctors or nurses. We are trained professionals and we can help connect you to the right people and services for your needs.

What We Offer

Community Health and Wellbeing Workers (CHWWs) are here to support patients with everyday challenges. Whether it's managing health, finding local services or just having someone to talk to, our team are here to help. And you don’t need a referral or a diagnosis. We work locally, in your neighbourhood, offering friendly and practical help that puts patients first.
