Community Health and Wellbeing Workers (CHWWs) play a key role in bridging the gap between health services and the people they serve. We work alongside NHS partners to provide practical, person-centred support that addresses the wider factors affecting health, helping people stay well and reducing unnecessary pressure on the system.

How We Support Primary Care

Our presence in practices and communities allows for early, informal intervention before problems escalate.

We work closely with GPs, PCNs, and wider primary care teams

  • Support patients with non-clinical needs such as isolation, housing issues, or low-level mental health concerns.
  • Reduce frequent GP attendances by addressing underlying social factors.
  • Increase patient engagement and follow-through with services and advice.
  • Connect individuals with local resources to build long-term independence.
  • Promote health campaigns and preventive care in the community.
