Who We Are, What We Do & Why We Care

We support individuals and families with their health, wellbeing, and general day-to-day challenges. Our approach is personal, respectful, and based on trust.

We work directly with people in the community. You’ll find us linked with GP surgeries, libraries, community centres, and local hubs. We're not tied to desks.

We're out and about, listening to people, building relationships, and offering a helping hand.

Our goal is not only to support patients, but also to support the wider system by reducing demand on key healthcare services.

What Makes Us Different?

We don’t just hand out leaflets or recommend you go on an app that you’ll never use again. We might signpost you to a local fitness class, or support class, and we would follow it up. We stick with you.

If you need help filling in a form, we’ll sit with you and go through it. If you're feeling lonely or anxious, we’ll take time to listen. If you’re not sure where to go next, we’ll help you figure it out.

Sometimes the solution is quick. Sometimes it takes time. Either way, we’re by your side.

Who We Work With

About us CHWWcommunity health and wellbeing worker

We’re part of a wider healthcare team that’s usually doctor-led at a group of GP surgeries, called Primary Care Networks (or PCN for short).

We liaise with GPs, social prescribers, mental health workers, first contact physiotherapists, council services or local charities. We don’t replace them, we connect the dots, so things don’t fall through the cracks.

We also work with volunteers, carers, and community leaders who share our goal of helping people feel healthier, more confident, and more connected.

Who Covers the Cost?

Generally speaking, we’re funded by the Integrated Care Board, although sometimes we’re also funded by the local hospital NHS trust as part of re-enablement programmes.
