Community Health and Wellbeing Workers are a vital part of the wider health and care system. We work alongside Integrated Care Boards (ICBs), GP Federations, and Primary Care Networks (PCNs) to strengthen the link between clinical care and the community. Our role is to take pressure off frontline services, reduce health inequalities, and improve access for people who might otherwise fall through the gaps.
We help build a stronger community infrastructure by connecting people to local groups, services and other available support. This community-first approach complements the goals of integrated care and helps shift the focus from reactive to proactive support.
We collaborate with social prescribers, care coordinators, mental health workers and others within Additional Roles Reimbursement Scheme (ARRS). A hands-on, relational contributions, focused on long-term engagement, often acting as a trusted link between services and person.
CHWWs are well placed to reach disadvantaged communities, engaging with individuals from underserved groups, with personalised support and promoting health literacy. Supports PCNs and ICBs health equity priorities and population health management.
We work with people who often need time, support and signposting — not necessarily a medical solution. By addressing the wider social and wellbeing issues behind repeated GP visits, we help free up clinical time.
We collect data and case studies to show our impact, whether it’s reduced GP visits, improved mental wellbeing, or increased community participation. Supports local systems to demonstrate value and key targets.

Whether it’s help understanding your health, finding local services, or just someone to talk things through with, we’re here to listen and guide you. Our support is flexible, friendly, and built around what matters most to you.
You don’t have to figure things out on your own. We know what’s available in your area and can help you access support like housing advice, benefits guidance, mental health services, carers’ support, and more. If you’re unsure where to go, we’ll help you get there.
We take time to understand your situation. Whether you’re facing a specific problem or just feeling a bit lost, we’ll listen and work with you to find the right next steps. This support is flexible and led by you, we can meet at your home, in a community space, or else where that feels comfortable.
We run pop-ins, cardiac fitness classes, and regular sessions to help people live healthier lives. We signpost to walking groups, healthy eating hubs, stress management, or support to stop smoking. It’s not about preaching, but practical, realistic changes that work.
